Getting an Immunization

Appointments are required for immunizations. **We do not offer travel immunization services.**

Please call 328-2402 Monday through Friday between 8:15 a.m. and 4:00 p.m. to schedule an appointment.  On the first, third and fifth Wednesdays, the phone line is open until 6:00 pm.

Clinic Hours

The Immunization Clinic is open Monday through Friday from 8 a.m. - noon and 1 p.m. - 4:30 p.m., excluding holidays.

Appointments are required for immunizations. Walk in appointments are limited to schedule openings.

Immunization Fees

  • Childhood immunizations
    • Fees vary depending on Vaccines for Children (VFC) eligibility. The following children are considered VFC eligible:
      • Children enrolled in Medicaid (Proof of enrollment in Medicaid is required at time of service.)
      • Children enrolled in Nevada Check Up (Proof of enrollment in Nevada Check Up is required at time of service.)
      • Children who are American Indian/Alaskan Native
      • Children who have insurance which doesn't cover immunizations
      • Children who do not have insurance
    • We accept Medicaid and Nevada Check Up 
  • Adult immunizations
    • We accept Medicaid
    • We do not accept Medicare, Senior Care Plus or Senior Dimensions.
  • We accept cash, checks, debit cards, or credit cards. We do not accept out-of-state checks.

Necessary Forms & Documents

Bring all immunization records for the individual(s) receiving immunizations. Our immunization registry may not contain immunizations given at other clinics, private physicians, or the school district. It does not contain records of immunizations given out of state.

We strongly encourage parents or court-appointed legal guardians to personally accompany their child for immunizations. There are forms, questionnaires, and consents that need to be read, completed, and signed by the parent or legal guardian.

Written authorization from the parent or legal guardian of the minor is acceptable but the minor must be accompanied by another adult. The minor cannot be alone.
