18 years and under WITHOUT insurance or have Medicaid:
If you do NOT have insurance or do have Medicaid, please print and fill out this form here.  Bring them, and your Medicaid card if you have Medicaid, with you to the event.  

19 years and over WITH any insurance (INCLUDING Medicaid):
If you have health insurance (INCLUDING Medicaid), please print and fill out this form here. Bring them and your insurance cards with you to the event.

19 years and older WITHOUT insurance:
Please fill out the form on site and bring them and your ID with you. (Limited vaccine types)
